Employees and their dependents are eligible for health care coverage beginning the first day of employment with the City of Lewiston. The City covers 100% of the premium for the employee and the employee pays 20% of the premium for dependents. Proof of dependency, such as a marriage certificate, birth certificate, etc. is required for dependent enrollment. Enrollment changes due to qualifying life events must be made within specific time frames dependent on the qualifying life event. Employees should contact HR if they have questions about enrolling dependents.
- Deductible is $1,500 per individual or $3,000 per family
- Maximum Out-of-Pocket is $3,000 per individual or $6,000 per family
- Doctor visit - $30
- Specialist visit - $50
